To check out books and videos from the library you need a library card. The form below is the application for a library card, you can fill most of it out using Adobe Reader and print it, or you can request one when you are at the library. If you are under 14 years old you'll need your parent's signature, which needs to be signed in front of library staff.

We do not give out actual library cards, we assign you a number and all you have to do is tell us your name when you are ready to check out.
